CMSA WAIVER – If you played indoor this season you do not need to complete another waiver. If you did not you will need to complete the 2022/2023 waiver for the first time.
Please complete the Waiver and return it to me asap so I can upload it for you.
CMSA 2022-2023 Season Player Waiver Form FULL

CMSA Proof of Birth – IF you are new to playing CMSA League they will also need proof of age. Please include a copy of the players Birth Certificate in your email to me.

CMSA Player ID Cards – All U11 and higher players will need a ID Card – If you haven’t not had one, have lost it or it has expired you will need to get one!

* CMSA will be at Calgary West Soccer Centre – Saturday, April 9 and Sunday, April 10 10:00am – 5:00pm. All other dates will be in the south of Calgary at the CMSA office.
